Remove errant Makefile and pm_to_blib from repo

Makefile and pm_to_blib were added to the git repository by commit
ddbedbfc2f. Having those files in the repository
causes problems with updating to Master in dev-mode installs. This patch removes
those two files, since they shouldn't be tracked anyway.
