tweak display of issuing library on staff reading history

Following idea in patch submitted by Kyle Hall on 1 April 2010,
use issues.branchcode instead of issues.issuingbranch to set
displayed issuing library. Also tweaked wording.

Bug 3606: fixes 'last 50 items only' link in members/readingrec.tmpl.
...
If a staff member clicked on 'Show all items' in a patron's reading record, and
then clicked on 'show last 50 items only,' all of the records would still be
shown. The link for the 'last 50' items contains a switch for 'limit=full'
instead of 'limit=50'.
Also fixes some XHTML errors.

Enabling AutoComplete on resident circ search form. Putting dependent YUI files into doc-head-close.inc along with button/menu-related YUI js and css. Moving menu.css, button.css, and dependent graphics into lib/yui. Removing "yui-skin-sam" class from body tags and altering css accordingly.

Continuing integration of breadcrumbs and resident search form, now on member pages. The search form tabs need to be handled in a more sane manner given that they might have multiple options. Either a custom handling script for the different includes or a js-library-based system that will handle all of them.
