This patch converts the toolbar on the suggestions page to to Bootstrap,
replacing YUI button code with Bootstrap markup.
To test, view the suggestions page and confirm that the "New purchase
suggestion" button looks correct and works correctly.

All tests were done with all 3 patches from this bug applied:
1) Add suggestion from OPAC, check copyright date is saved
2) Add suggestion in intranet, check copyright date is saved
3) Edit suggestion, check change is saved correctly
4) Check date filters work as expected, using single dates, date ranges
and multiple filters at once:
- Accepted on
- Managed on
- Suggested on
5) Check copyright date search works correctly.
Passes all tests and QA script.

Main patch correcting suggestions search :
- allow filtering on copyrightdate, manageddate and accepteddate
- entered dates remains in filter during search
- empty and null values are in the same tab
- null/empty values are not present in filers comboboxes

All instances of the old DynArch calendar have been replaced with
jQueryUI versions and the old library files have been removed.
calendar.inc has been modified to include jQueryUI localization
strings and global configuration options. Just add a "datepicker"
class to an input field to trigger a datepicker prompt.
If you would like two fields in one from to limit each other (one
is date from, one is date to), add these classes to each:
"datepickerfrom" and "datepickerto." This will prevent an invalid
entry, e.g. a date in the latter which falls before the former.
jQueryUI is now upgraded to the latest verision, 1.8.21.
Edit: Now with proper translatability, date formatting, first day
of the week handling, and RTL support.

Current jQuery-driven tabs are done using a very old
version of the tabs plugin. This patch upgrades jQueryUI
to the latest version and adds the tabs widget dependency
to the jqueryui js file and updates the syntax for existing
tabs:
- $("#foo > ul").tabs(); changes to $("#foo").tabs();
- Remove full URL from tab links (use #anchor only).
Pages with "static" tabs (tabs which are built in the
markup rather than generated by the plugin) have been
modified to use their own style. Examples: pay.tt in
the staff client and opac-readingrecord.tt in the OPAC.

In some places the suggestions were 'pending' in others the same status
was named 'pending'. This patch changes the status to 'pending' in all
places on the suggestion page to make it more consistent.

This patch makes the tabs on the manage suggestion page translatable.
To test:
- Create some suggestions in intranet or opac and manage them to make the
different status tabs appear: accecpted, pending, checked, rejected
- Apply patch, run 'perl translate update <languagecode>'
- Check po file for string '%s %sPending %sAccepted %sChecked %sRejected %sAvailable %sOrdered%s %s%s %sNo name%s (%s)'
- Translate it, save and run 'perl translate install <languagecode>'
Tabs should now appear translated.

There are a couple suggestion statuses referred to
in the code and suggestion management page that currently
do not have notice templates defined: ASK and CHECKED. These
seem to be internal, so perhaps default message templates
are not needed for these.

Displays the Branch name and Category description along with their codes in Suggestion Management.
Also since I joined to the branches table the 'map' function in opac-suggestions.pl to retrieve the branchname was no longer necessary.

A couple of enhancments to Suggestions Management.
Adds a display of the borrower category of the suggesting patron for each item.
Adds links to the title and author displays so that the catalog can be quickly search for an existing bib.
