TYPEDOC authorised value category is needed to permit

This follow-up makes two additions:
- The UPLOAD category is added to auth_val_cat.sql for new
installations.
- A description of the category is added to the authorized values
template.

- This patch provides an illrequests.status_alias column that is a foreign key to
authorised_values.id. This provides the ability for an ILL request to
have an optional status alias that is defined as an authorised valued.
- A new ILLSTATUS authorised value category is also made available during install.

Changes were made to address QA issues from Comment 17
Tests:
1) Verify that a new entry for CONTROL_NUM_SEQUENCE is added to table authorised_value_categories
2) Edit a bib record using advanced editor and note that the 001 widget isn't there when adding/editing 001 field
3) Add a new row into authorised_values table with:
a) CONTROL_NUM_SEQUENCE in category column
b) authorised_value column has a string ending with a number i.e. sprLib0001
c) lib column has a short string indicating the type of control number i.e. "Springfield Library"
4) Edit a bib record using the advanced editor and note that the 001 widget is there when adding / editing 001 field

The ROADTYPE category was missing from the default list of
authorised value categories available.
To test:
- Verify ROADTYPE is not listed as authorised value category
- Apply patch and run database update
- Verify ROADTYPE is now listed
- Add an entry, for example: AV = Avenue
- Add or edit a patron, verify a pull down with your entry
shows on the form.
