Biblio.pm bugfixes

- uses new C4::Dates instead of C4::Date
- start work on TransformMarcToKoha
  * handle ambiguities in certain column names
  * part of patch inspired by Chris Cormack
- fix bugs preventing biblioitems and items
  from being properly updated
- ensure that biblionumber and biblioitemnumber
  always get mapped to MARC
