The error pages wrote a HTTP status code of 200 for all PSGI requests, even
though it should have only done it for PSGI requests from the ErrorDocument
middleware. This patch fixes that.

On bug 17591 we discovered that there was something weird going on with
the way we export and use subroutines/modules.
This patch tries to standardize our EXPORT to use EXPORT_OK only.
That way we will need to explicitely define the subroutine we want to
use from a module.
This patch is a squashed version of:

There is a "debug" parameter we are passing from the controller scripts
to C4::Auth::get_template_and_user, but it's not actually used!

This patch uses the ErrorDocument middleware to use Koha's
custom error pages instead of generic Plack error responses

This patch changes the 400, 401, 402, 403, 404, 500 error pages to be handled
by only one template for staff and only one template for OPAC.
Additionally it removes a translatability issues due to sentence splitings by html tags.
To test:
- Apply patch
- Trigger 404 error for staff and for OPAC by calling a page that does not exist
- Try to trigger other error pages and/or carefully review code changes
in the *.pl files
- Review koha-tmpl/opac-tmpl/bootstrap/en/modules/errors/errorpage.tt and
koha-tmpl/intranet-tmpl/prog/en/modules/errorpage.tt

Since we switched to Template Toolkit we don't need to stick with the
sufix we used for HTML::Template::Pro.
This patch changes the occurences of '.tmpl' in favour of '.tt'.
To test:
- Apply the patch
- Install koha, and verify that every page can be accesed

The scripts did not define the status printed on the first line of
HTTP headers correctly: they were sending 200 OK,
which was confusing for browsers.
